A case of lymphedema associated with juvenile rheumatoid arthritis

Abstract
Lymphedema is classified into primary and secondary types. Primary lymphedema includes conditions such as agenesis or hypogenesis of lymph vessels. Secondary lymphedema usually follows trauma, infection, malignant condition, radiation injury, or any of a number of connective tissue disorders, including rheumatoid arthritis. Lymphedema is an uncommon extra-articular feature of rheumatoid arthritis (RA) and is especially uncommon in juvenile rheumatoid arthritis (JRA). The lymphoscintigraphic findings of JRA are different from those of other conditions. We report the case of a 13-yearold boy with right lower leg lymphedema associated with the systemic type of JRA. The lymphoscintigraphic findings of JRA show absence of functioning lymphatics in the right lower limb. These findings show that lymphedema in JRA is due to lymphatic obstruction such as would be caused by chronic lymphangitis, abnormal fibrinolysis, or deposition of circulating immune complexes in lymph vessels.
